Enclosed are forms on which you may submit a report
concerning your COP OLLMEN during the present term.

Will you fill out one form for each class, indica-
ting the number of students from each of the different
schools of the University which make up the class.

In the case of classes in which sudents may be en-
rolled for varying amounts of credit, please indicate
at the bottom of the sheet how many students are en-
rolled for one hour of credit, two hours of credit,
three hours of credit, etc.

Please return these at your earliest convenience.
