Heads of Departments:

I am enclosing blanks which the army requested we fill

out for each instructor or assistant in the army program.

I believe that the blanks are self-explanatory, in the
last colum in the number of ASTP sections taught, it
would be well to indicate whether the person is the
laboratory assistant, assistant quig section instructor,
or instructor.

These should be returned at your earliest convenience
for this report is supposed to go to Omaha at the end
of the 4-week term,
