This is to authorize Dr. Forpast é ATién, of Lawrence, Kansas to

act as our representative to sell our Normalizers either retail or
wholesale anywhere in the U.S. Providing however that the then pre-
-vailing retail ané wholesale prices must be adhered te strictly.

As compensation for each retail sale Dr. Allen will be paid 33 I/3 %
of the reatil sales price. Compensation for sales wholesale to be
set later at such a rate as shall be mutually agreeable.

Dr Allen is to be entitled to compensation when any such sales price
is collected by this company. A commission shall be due Dr. Allen if
any sale is made by us thru leads furnished by him.
