Our records show that you were issued operator's license No. 16-7764. We now find that you have mde a second application te this current period, this application being No. 16-16368. We will,therefore, ask that you return receipt No. 16-16368, which was given you at the time second applica- tion was made, in order that we may authorize a refund of the 40-cent fee which accompanied this application. Enclosed is an application for a duplicate. If the photostatic copy of your original license has been lost or destroyed and you desire a duplicate, please complete the form and return to this office. The regular fee for a duplicate being 25 cents, we will deduct this amount from the 40 cents remitted for your second application, and the 15-cent refund will be authorized and mailed you in due time.
